Income-Related Monthly Adjustment Amount for Medicare Beneficiaries (IRMAA)
What is IRMAA?
IRMAA stands for Income-Related Monthly Adjustment Amount. It's an additional charge added to Medicare Part B and Part D premiums based on your income. If your income exceeds a certain threshold, you’ll pay a higher premium for your Medicare coverage. IRMAA was passed as part of the Medicare Modernization Act of 2003. The higher premiums for higher-income beneficiaries began in 2007. The income thresholds and the corresponding IRMAA amounts change annually.
How is IRMAA Calculated?
IRMAA is based on your modified adjusted gross income (MAGI). The Social Security Administration (SSA) will notify you if you owe the IRMAA surcharge, but you can appeal if your financial situation changes. For example, if you amend your tax return and it affects your MAGI, inform the SSA with a copy of the amended return and your IRS acknowledgment receipt.
Your MAGI is recalculated yearly, so your IRMAA may vary based on your income. For 2025, your IRMAA will be based on your 2023 MAGI, similar to how your 2022 MAGI determined 2024 IRMAA.
To calculate your Medicare-specific MAGI, start with the adjusted gross income (AGI) on line 11 of IRS Form 1040. This includes taxable income like IRA withdrawals, capital gains, and taxable Social Security benefits. Then, add any tax-exempt interest from line 2a of the same form.
